To facilitate timely communications to the ACCC Membership and in compliance with the AKC requirement regarding electronic communication, the ACCC Board needs to have EACH Applicant’s written consent. Those who choose to not receive Email, or other Electronic Communication updates from the ACCC will receive Bylaw required notices sent by USPS mail. The use of Electronic Communication is an effort to not only keep costs down but to notify our members quickly and efficiently, and to be good stewards of the environment.

I, THE UNDERSIGNED, hereby apply for membership in the American Chinese Crested Club, Inc., and hereby agree to abide by the Constitution and By-Laws of the American Chinese Crested Club, Inc. and by the rules and regulations of the Club and the American Kennel Club. I have read and truthfully answered all questions on this application. I have read and agree to abide by the American Chinese Crested Club, Inc. Code of Ethics. I further understand that if I breach the Constitution and By-Laws and/ or the Code of Ethics such breach would be probable cause for dismissal from the Club.
Should my application be denied, the Club is not obligated to make the reason known.

By signing this waiver, you, the undersigned parent or legal guardian, acknowledge and agree to the following terms regarding your minor child’s participation in activities and events organized by the American Chinese Crested Club (ACCC):

1. Parental Consent: I, the undersigned, give my consent for my child to participate in all events, activities, and functions associated with the American Chinese Crested Club (ACCC).
2. Responsibility for Supervision: I understand that I am solely responsible for ensuring my child is always supervised during ACCC events. I agree to make the necessary arrangements for my child’s supervision while they are participating in any ACCC activities.
3. Compliance with Rules and Regulations: I acknowledge that it is my responsibility to ensure my child understands and follows all ACCC rules, regulations, and guidelines set forth for participation in events. I further agree to assist the ACCC in maintaining a safe, positive, and rule-compliant environment by ensuring my child’s proper behavior and conduct.
4. Waiver of Liability: I understand and agree that the American Chinese Crested Club, its officers, members, and event organizers are not responsible for any accidents, injuries, or incidents that may occur during participation in any ACCC event. By signing this waiver, I release the ACCC from any liability, claims, or damages arising from my child’s participation. By signing below, I acknowledge that I have read, understand, and agree to the terms and conditions outlined in this waiver.
